Key Responsibilities
- Provide consultations for clients and collect data to monitor progress.
- Deliver a range of high-quality physical activity opportunities that are progressive and reflective of the needs of special populations in the local community.
- Implement strict data and management protocols including GDPR compliance.
- Assist in the evaluation of NERS by collecting accurate data.
- Signpost clients to appropriate exit routes.
- Support any new Exercise Referral Professionals and ensure they understand their role in the scheme.
- Attend and represent the NERS Coordinator at quarterly meetings if required.
- Ensure the development and dissemination of good practice in exercise delivery by researching evidence regarding exercise and medical conditions.
- Provide cover for other Exercise Referral Professionals where necessary.
- Deliver fitness classes and maintain a safe and welcoming environment at Knighton Sports Centre.
Administrative Duties
- Telephone communications.
- Distribution of appropriate mail.
- Booking appointments.
- Monitoring scheme attendance and client programmes.
- Ensuring client follow-up procedures are implemented.
- Collate information regarding the scheme and share it with the Coordinator.
- Maintaining accurate and up-to-date records on the NERS Theseus system, the official patient and participant management database, including monthly monitoring and reporting in line with Public Health Wales requirements.

- Registered at Level 3/4 CIMSPA.
- Walking Way to Health leaders training.
- Defibrillator training.
- Experience of working on a GP Exercise Referral Scheme or a Chronic condition rehabilitation scheme.
- Welsh Language skills are desirable.
