SUPPORT PLANNER - RSAP NORTH LINCOLNSHIRE, SCUNTHORPE/LINCOLN,PERMANENT, 37HRS, GRADE 3, £25,235, FTE

You may want to work with others to help people create a better future for themselves, if so, we have an opportunity for the role of Support Planner in the RSAP North Lincolnshire service that could be just what youre looking for.
We provide accommodation for those rough sleeping/sofa surfing either in a hostel or dispersed environment. Our aim is to meet their support needs and help the individual overcome challenges which could result in further homelessness.
Duties and Responsibilities
- To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
- Be responsible for assessing and evaluating the needs of Service Users.
- To ensure that tasks identified in support plans are communicated to Support Workers and that these tasks are clear, appropriate and agreed with the Service User.
- To enable Service Users to understand the requirements of independent living including budgeting, maintenance and use of leisure time.
- To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
