SUPPORT PLANNER – LHRSP FLOATING SUPPORT, EAST LINDSEY, PERMANENT, 37HRS, GRADE 3, £25,235 FTE

If so, we need you to help us support adults in our East Lindsey Floating Support service. As a Support Planner you will be the lifeline of Floating Support, where every day will be different, both challenging and rewarding. You will work with Service Users, empowering them to move on to live independently. No experience needed; we provide full training. Above all, we want your attitude, personal qualities, transferrable skills and willingness to be a part of our hard working and determined team. We are looking for reliable and self-motivated candidates who can demonstrate an understanding of the issues facing those affected by homelessness.
Duties & Responsibilities
- Sign post Tenants onto appropriate services for substance misuse management & health.
- Assist service users with housing applications and move on options.
- Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
- To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
- To assess, co-ordinate and review the planning and delivery of support to service users
- To be responsible for assessing and evaluating the needs of service users, recording relevant information specific to a persons support needs and co-ordinating referrals into the service.
- To motivate and work creatively with service users to engage with their individual support/ resettlement plan
