Job Description SUPPORT PLANNER – LHRSP PATHWAYS CENTRE LINCOLN, PERMANENT, 37HRS, GRADE 3CORE, £25,235 , FTE Do you have some Support Work experience or lived experience? We provide accommodation for those rough sleeping/sofa surfing either in a hostel or dispersed environment. Our aim is to meet their support needs and help the individual overcome challenges which could result in further homelessness. Duties and Responsibilities To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs. Be responsible for assessing and evaluating the needs of Service Users. To ensure that tasks identified in support plans are communicated to Support Workers and that these tasks are clear, appropriate and agreed with the Service User.
