We are looking for an enthusiastic and committed person to join the Nottingham Recovery Networks Recovery Case Management Teams. The role will have its own caseload and support individuals who have substance use issues including alcohol and prescribed medications to manage their dependent drug use, usually OST, and who need recovery related support, advice and guidance.
As you will be the next point of contact following Triage, working with people early in their treatment journey, people and motivational skills will be essential to make clients feel safe, comfortable and reassured that they are in good hands. The successful candidate will use their experience to provide high quality evidenced based substance use treatment to adults with dependence on substances in line with national guidance. This will include 1-2-1 advice and structured treatment.
The post holder will proactively maintain contact with service users striving for positive outcomes for all to include where possible the attainment or maintenance of abstinence. The post holder is required to be competent and autonomous in dealing with more complex cases and will have the experience and knowledge to competently advise more junior members of the team.
Key Responsibilities:
- To provide highly effective recovery case management. This will include effective management of service users who are prescribed OST or require medication to address alcohol dependence.
- To carry out highly effective 1-2-1 interventions to support clients towards their recovery goals.
- To formulate appropriate and structured care plans ensuring service users receive evidence-based interventions and assist in the delivery of structured care plans.
- Key working with service users to build successful and trusting relationships and providing Harm Reduction interventions.
- To maintain appropriate records and case notes in line with the service outcomes.
- Care planning, risk assessment and risk management.
- Have responsibility for supporting and mentoring more junior members of staff.
- Participation in MDT forums for supervision, learning and development.
The successful candidate for the role of Senior Substance Use Worker will have: At least a Level 3 Diploma in Health.
