About the Role:

We are currently seeking a passionate and motivated Outreach Family Support Worker to join our dedicated team. In this vital role, you will work directly with children and families, carrying out Early Help Assessments, developing tailored support plans, and coordinating multi-agency interventions to ensure the best outcomes for all family members.

Key Responsibilities:

  • Undertake Early Help Assessments with families to identify strengths, needs, and areas for support.
  • Develop and implement family-centred support plans in collaboration with multi-agency partners.
  • Lead and chair Team Around the Family (TAF) meetings to regularly review progress and adapt plans as needed.
  • Build strong, trusting relationships with children and young people to explore their views, wishes, and concerns.
  • Ensure that interventions are impactful, child-focused, and contribute positively to long-term outcomes.
  • Adopt a whole-family approach to addressing challenges and promoting resilience.

What We Are Looking For:

  • Experience working with children, young people, and families in a support or safeguarding capacity.
  • Strong assessment, planning, and coordination skills.
  • Ability to engage effectively with multi-agency professionals and services.
  • A compassionate, empathetic, and non-judgmental approach.
