Four Oaks Healthcare is a nurse-led domiciliary care and, due to expansion, we are looking for an enthusiastic, passionate individual to join our growing team as a care coordinator.
As the care coordinator, you will be responsible for managing a team of care assistants as well as maintaining a good communication line with all customers and other parties to ensure we deliver a high standard of care.
The ideal candidate will:
- Have a passion for delivering good quality, person-centred care
- Ideally have at least 2 years of experience within the domiciliary care sector
- Be flexible and reliable
- Be computer literate
- Be a good team player
- Be motivated and able to work on their own initiative
- Have exceptional people management skills
- Must be a driver with access to own vehicle
Duties Involved:
- Preparing & scheduling rotas
- Writing and reviewing care plans
- Conducting risk assessments
- Interviewing potential candidates
- Conducting spot checks
- Being on call
