This community-based role involves delivering one-to-one physiotherapy care in patients’ homes to facilitate early supported transfer from hospital or prevent unnecessary admission. You will build lasting relationships with patients, stakeholders and support networks to aid their treatment and recovery.

Typical day will include:

  • Conducting comprehensive holistic assessments of patients
  • Formulating individualized physiotherapy treatment plans
  • Delivering physiotherapy sessions in home or community settings to maximise patient independence
  • Educating patients and carers on the correct use of mobility and functional equipment

Requirements:

  • Previous community experience or a track record of working autonomously
  • Experience in an acute care setting
  • HCPC registration
  • Full UK driving licence
