Fountains Medical Practice has a growing practice population and is based in Fountains Health. In addition to providing an excellent primary care service to our patients, we provide a successful service to all the care homes in Chester city through the Fountains Integrated Care Homes Service (FICHS). The FICHS team works with the multidisciplinary services in the community, secondary care specialists and voluntary sector to provide holistic care to patients in care homes. We work closely with other practices as part of a successful and expanding PCN and we are a training practice, working with Medical Students from Chester University and the University of Buckingham.
Main duties of the job
Due to continued expansion, an exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to join our Care Home Team. We would ideally expect the successful candidate to have experience of managing patients with long term conditions, acute illnesses, a working knowledge of QOF and the current Direct Enhanced Services. Experience or interest in care of the elderly and palliative care is desirable. Applications are welcome from both newly qualified and experienced ANPs. The candidate must have excellent clinical and interpersonal skills to ensure safe, effective and high standards. They should be willing to work hard to develop the role to complement the clinical and administrative support staff.
