Type: Permanent About The Role The Forward Trust is seeking a compassionate and motivated Health & Wellbeing Practitioner to join our team at HMP Send, Surrey, working 18.75 hours per week.

As part of our integrated Health & Wellbeing Service, you'll support women experiencing substance misuse and low-level mental health challenges, helping them build healthier, more positive futures within a trauma-informed and gender-responsive environment.

Working as part of a multi-disciplinary team, you'll deliver recovery-focused interventions, provide practical support, and help service users prepare for successful reintegration into the community.

What you'll be doing

  • Managing a caseload and coordinating individual support plans
  • Conducting assessments and developing person-centred care plans
  • Delivering structured one-to-one sessions and facilitating group programmes
  • Providing harm reduction advice and guidance
  • Supporting release planning and through-care arrangements
  • Working closely with healthcare, mental health, and prison colleagues
  • Maintaining accurate records and contributing to service performance targets
  • Undertaking second signatory duties and oral swab testing where required What we're looking for:
  • Experience supporting individuals with substance misuse, mental health, or complex needs
  • Excellent communication and relationship-building skills
  • A proactive, organised, and solution-focused approach
  • Confidence working within a multi-disciplinary environment
  • Strong IT and record-keeping skills
  • Commitment to completing the Drug and Alcohol Workforce Development Programme (DAWFDP) You'll be part of a dedicated team transforming lives every day, with access to ongoing training, professional development, and the opportunity to make a lasting impact in a unique and rewarding setting.

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting.

These checks can take up to 5 months to complete.

Any offer of employment will be conditional upon the successful completion of both checks.

Please note if you have lived overseas within the last 5 years then checks may take longer.
