The Forward Trust is seeking a Health and Wellbeing Practitioner to support adults affected by drug and alcohol use. You will manage a caseload, carry out comprehensive assessments and deliver structured psychosocial interventions in a multidisciplinary team.
You will provide harm reduction guidance, develop SMART care plans, and deliver both one-to-one and group interventions tailored to individual recovery goals. Flexibility includes possible evenings/weekends and travel within the cluster.
