Forward Trust is seeking a Mental Wellbeing Practitioner to join the team at HMP Chelmsford, supporting adults with co-existing mental health and substance misuse challenges. You will deliver one-to-one and group interventions, conduct assessments, and help develop care plans within a multidisciplinary prison environment.

You will build relationships with partner agencies, ensure accurate records, and contribute to service delivery targets and positive outcomes for service users.
