The Forward Trust is looking for a Continuity of Care Coordinator to join the substance misuse team at HMP Brixton. Based primarily at Brixton, you will support the prison-to-community care pathway and collaborate across London services, including Wormwood Scrubs and Heathrow IRC.
You will lead audits, share critical information with community teams, and help ensure timely referrals and improved outcomes for those leaving custody.
