About The Role
Are you passionate about helping people make lasting change? We’re looking for a Senior Health & Wellbeing Practitioner to join our Integrated Substance Misuse Team at HMP Usk, Prescoed & Canol, working full time (37.5 hours per week). This is a rewarding leadership role supporting adult males within a prison setting, combining frontline clinical work with team management and service development. You’ll play a key role in supporting individuals with substance misuse and low-level mental health or emotional wellbeing needs, helping them progress through structured recovery journeys and achieve positive outcomes.
What you’ll be doing:
- Line managing Health & Wellbeing Practitioners and supporting their development
- Managing a varied caseload and delivering 1:1 interventions and group work
- Carrying out assessments, care planning and appropriate referrals
- Working closely with healthcare teams and multi-agency partners
- Supporting service performance, outcomes and reporting requirements
- Promoting equality, diversity and inclusive practice across all work
What we’re looking for:
- Experience in substance misuse, mental health or wellbeing services
- Strong leadership and supervisory experience
- Confidence delivering both group and individual interventions
- Ability to build effective working relationships across agencies
- Organised, proactive and solution-focused approach
- Commitment to high-quality, person-centred support
- Welsh and English speaking preferred

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete.
