About The Role
We're looking for a passionate and proactive Health and Wellbeing Practitioner to join our team at HMP Cardiff on a full-time basis (37.5 hours per week). This is an opportunity to support individuals on their journey towards positive change, helping them improve their health, wellbeing, and prospects for a successful return to the community. You'll manage a varied caseload, deliver tailored interventions, and work collaboratively with service users to develop meaningful care and release plans that support long-term recovery and wellbeing.
What You'll Be Doing
- Managing a caseload of service users and coordinating their treatment journeys
- Conducting assessments and developing person-centred care plans
- Delivering structured 1:1 interventions and facilitating group sessions
- Providing harm reduction advice and wellbeing support
- Supporting release planning and through-care arrangements for individuals returning to the community
- Carrying out second signatory duties and supporting clinical services
- Working closely with multidisciplinary teams and partner agencies
- Maintaining accurate case records and performance data to meet service targets
