Fortcom Health Services seeks a dedicated Per Diem Registered Nurse (RN) / LPN to support our Adult Family Care program. This in-home role offers flexible hours for an RN seeking meaningful community-based nursing care.
You will perform visits every two months and complete all required clinical documentation for a limited caseload. Collaborating with the Program Director and AFC team, you will develop AFC plans, supervise caregivers, review physician summaries, and coordinate member discharges.
