Overview: We have an exciting opportunity available for a Care Coordinator on a full-time basis, working from our head office in Sible Hedingham. Experience in working with those with learning disabilities is desirable but not essential.
Responsibilities:
- Ensure that high quality domiciliary care services are delivered to service users through efficient management within the contracted service area.
- Coordinate and prepare rotas, care packages and care staff on a day‑to‑day basis.
- Address developmental needs of care staff and performance levels through supervision (one‑to‑ones) and ensure observations of practice and other compliance tasks are completed according to target dates.
- Be on-call alternating weekends and attend calls when a shift cannot be covered.
- Supervise and support the care team in day‑to‑day service delivery to the highest possible standard, ensuring all visits are covered and none are missed or cancelled.
- Re‑allocate visits and rounds quickly, effectively and safely at all times, confirming instructions directly and by text to avoid misunderstandings.
- Ensure on‑call staff are telephoned or seen daily to be updated on hospital admissions, concerns about service users and general issues.
- Manage day‑to‑day visit changes, ensuring service is maintained to all users; perform rounds when capacity is challenged.
- Deal with service users’ queries and respond to them effectively and quickly.
- Notify service users or appropriate others in advance of any operational issues affecting their service delivery, including when more than 30 minutes late.
Requirements:
- Valid clean driving licence.
- Own reliable vehicle.
- Experience working with individuals with learning disabilities (desirable).
- Strong organisational and supervisory skills.
