Join an experienced care and clinical team as a Case Manager, helping to shape specialist homecare across the Chelsea area. What you’ll do * Champion the highest standards of care and service for every client. * Assess, support and supervise care assistants, including during induction. * Mobilise new care packages and make sure they consistently meet each client’s needs. * Contribute to Quality Improvement Plans and ongoing monitoring. * Take part in the shared out-of-hours on-call rota with the leadership team. What you’ll bring * Level 3 Health and Social Care qualification, or equivalent. * Experience delivering care and support services, with a track record of leading and supervising care teams in community or live-in care. * Experience completing risk assessments and personalised care plans. * Confident communication and IT skills.
