The predicted salary is between 36000 - 60000 £ per year.

Opportunity to join a small team of experienced and enthusiastic pharmacists and technicians.

Salary: Band 7-8a equivalent pro rata, however as not Agenda for Change salary will be based on previous experience.

Annual leave: 25 days pro rata plus bank holidays.

Working hours: Full time 30 hours per week.

NHS Pension: You will be fully supported with a comprehensive induction and enrolment on the compulsory Primary care pharmacy education program (if not already undertaken) to support and encourage career development.

Main duties of the job:

  • Undertake structured medication reviews for patients both in and out of care homes to proactively manage patients with chronic diseases and complex polypharmacy.
  • Support management of medicines on transfer of care, especially back into primary care.
  • Support the development and implementation of systems for safer prescribing and monitoring of medicines.
  • Provide specialist medicines management advice to patients and health care professionals.
  • Work with community and hospital colleagues to help resolve medication-related issues.
  • Work on, and lead on, large scale projects to improve medicines safety across the PCN.

About us: Avon Valley Primary Care Network is an umbrella organisation delivering care to 40,000 patients, across four GMS Practices based in the New Forest area. The Practices include The Fordingbridge Surgery in Fordingbridge, The Ringwood Medical Centre and Cornerways Surgeries in Ringwood and Twin Oaks Surgery in Bransgore. The practices have been working together for some time and have a good relationship. Together we aim to build robust services across our network to improve patient care and are keen to bring in new members of the primary care team to achieve these goals.

Job responsibilities: To be part of a multi-skilled primary care team in a patient facing role providing patient centred medical services based on patient needs with a focus on continuity of care.

Person Specification:

  • Knowledge and skills: Ability to provide quality care.
  • Qualifications: Pharmacy Degree, Registration with the General Pharmaceutical Council, Centre for Pharmacy Postgraduate Education primary care Pharmacy Education Pathway (PCPEP) qualification or working towards/willing to enrol, Registered non-medical prescriber, or willing to work towards appropriate qualifications.
  • Experience: Relevant theoretical and practical knowledge of Primary Care Networks, General Practice and evidence-based medicine, An appreciation of NHS agenda, national policies and government targets, An understanding of the current PCN DES specifications, A wide range of IT skills especially Microsoft 365 software and specifically Excel.
