We are currently seeking an enthusiastic and patient-focused Locum Prescribing clinical Pharmacist to join our well-established team at Fordbridge Medical Centre in Surrey. This is a full-time temporary position based in surgery Monday to Friday, 37.5 hours per week. Preferable is experience working in a GP setting, knowledge of Emis Web and Docman 10, and experience of long-term condition management clinics.
Main duties of the job:
- The post holder will provide expertise in clinical pharmacy and medicines optimisation as part of a multi-disciplinary team providing care for patients from general practice, community sites including care homes and patients' homes where required.
- You will be responsible for delivering care directly to patients, including those with single condition problems and long-term conditions, responding to medicine-related queries and medicine management on discharge/clinical letters.
- You should be trained in Controlled drug management.
- Ability to run the Prescribing Quality Scheme in the practice.
