Main duties of the job

As a Clinical Pharmacist, you will play a key role in optimising medication use, improving patient outcomes, and ensuring safe and effective prescribing. Working within your professional boundaries you will be responsible for delivering structured medication reviews, supporting chronic disease management and providing expert advice on medicines.

Job responsibilities

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for and undertake clinical medication reviews and support patients to proactively manage long term conditions.

Qualifications

  • Registration with the General Pharmaceutical Council (GPhC)
  • Accredited Master of Pharmacy degree
  • Clinical Pharmacy Diploma or Completed Primary Care Pharmacy education pathway
  • Disclosure and Barring Service Check
