Deliver care plans and follow individual clinical requirements. Support clients with complex medical and physical needs. Administer medication in accordance with prescribed instructions and relevant policies. Monitor and document changes in a client's condition. Provide clinical interventions appropriate to your training and competency. Work collaboratively with families, carers, community professionals and the wider healthcare team.
