About the Role
We are recruiting a Band 6 Community Dietitian to join the Community Adult Rehabilitation Service within CNWL. This is an excellent opportunity to work as part of a supportive multidisciplinary team, delivering specialist nutritional support to adults within their own homes, care homes, and intermediate care settings.
Key Responsibilities
- Provide specialist dietetic assessment and treatment for adults requiring nutritional support, including Home Enteral Tube Feeding (HETF).
- Deliver evidence-based nutritional interventions for patients who are malnourished, at risk of malnutrition, or enterally fed.
- Assess and calculate nutritional requirements, including food fortification, oral nutritional supplements, and enteral feeding regimens.
- Provide dietary counselling and support behaviour change to patients, families, and carers.
- Make recommendations to GPs regarding appropriate prescribing of oral nutritional supplements and enteral feeds.
- Participate in MDT meetings, discharge planning, and collaborative community care pathways.
- Maintain accurate clinical documentation and caseload management.
- Support training and education for healthcare staff, carers, and students.
- Contribute to audit, service development, and quality improvement initiatives.
Essential Requirements
- Degree or postgraduate qualification in Dietetics.
- HCPC registration as a Dietitian.
- Experience working within community or adult rehabilitation settings.
- Knowledge of nutritional support, including oral nutrition support and enteral feeding.
- Strong communication, clinical reasoning, and organisational skills.
- Ability to work independently and manage a clinical caseload effectively.
- Full UK driving licence and access to a vehicle for community visits.
Desirable
- Experience in Home Enteral Tube Feeding services.
- Experience supervising students or support staff.
- Knowledge of rehabilitation and long-term condition management.
- Experience contributing to audit or service development projects.
