About The Role
We are seeking an experienced Band 6 or above Registered Mental Health Nurse to join a dedicated Hospital at Home service. This is a rewarding opportunity to provide high-quality, one-to-one care for a 21-year-old male with Autism and Learning Disabilities within his own home. The successful candidate will be compassionate, confident working independently and experienced in supporting individuals with complex care requirements.
Key Responsibilities
- Provide dedicated one-to-one nursing care within the patient's home
- Deliver safe, individualised and person-centred care
- Support the patient's Autism and Learning Disability needs
- Monitor physical and mental health and identify changes requiring clinical intervention
- Follow individualised care plans and clinical protocols
- Maintain accurate and up-to-date clinical documentation
- Build positive relationships with the patient, family and wider multidisciplinary team
- Promote dignity, independence, safety and wellbeing
- Work collaboratively with healthcare professionals to ensure continuity of care
What We're Looking For
Essential
- Current NMC Registration
- Band 6 or above nursing experience
- Previous experience supporting individuals with Autism and/or Learning Disabilities
- Experience delivering one-to-one patient care
- Excellent communication and interpersonal skills
