A busy Primary Care Network in Leeds is recruiting an experienced Home Visiting Nurse / Nurse Practitioner to deliver same-day acute domiciliary visits and provide proactive frailty reviews for housebound patients across the local community. This is a varied, autonomous role with strong clinical support around you — including regular debriefs, access to prescribing ACPs, a GP on-call for advice, and opportunities to work within a well-established multidisciplinary frailty service.

The Role
- Acute Home Visiting (Reactive)
- Assess and treat urgent conditions at patients’ homes (e.g., UTIs, chest infections, oedema, infected wounds, migraines, exacerbations of long-term conditions)
- Provide treatment plans, safety-netting, and referrals (including pathways such as Virtual/Home Ward where appropriate)
- Prioritise and manage your own visiting schedule, including arranging hospital admission if needed
- Maintain accurate clinical documentation for every consultation
- Deliver holistic annual reviews for patients living with frailty
- Complete Comprehensive Geriatric Assessments (CGA) covering physical, cognitive, functional and social needs
- Contribute to weekly MDT discussions alongside clinicians such as a consultant geriatrician, specialist nursing, and a pharmacist
- Support personalised care planning to help patients remain safely at home and reduce avoidable admissions
What We’re Looking For (Essential)
- NMC registered (Diploma or Degree in Nursing)
- Independent Nurse Prescriber with minimum 3 years’ prescribing experience
- Strong experience assessing and managing acute presentations and long-term conditions
- Confident working independently in patients’ homes, with sound clinical judgement and clear escalation when needed
- Comfortable using clinical systems (e.g., SystmOne) and Microsoft tools
- Excellent communication skills with patients, carers, GPs and wider services
- Ability to travel across the community to complete visits
- Willingness to complete/hold a DBS check
- Advanced Clinical Practice qualification
- Experience building partnerships across organisations (including voluntary sector)
