We’re looking for an experienced, compassionate Care Coordinator to join a growing home care service in Oxford — someone who understands that great care starts behind the scenes, with thoughtful planning, strong relationships, and genuine attention to detail.

The Heart of the Role

As a Care Coordinator, you’ll be at the centre of everything — connecting clients, care staff, and the wider team to ensure care runs smoothly, safely, and with compassion. You’ll play a key role in supporting people to remain in their own homes, maintaining independence and quality of life through well-planned, person-centred care. What You’ll Be Doing

Your role will combine coordination, quality, and communication:

  • Care Planning & Quality
  • Writing and updating personalised care plans
  • Ensuring care is delivered in line with individual needs and preferences
  • Carrying out supervisions and supporting compliance standards
  • Monitoring quality and ensuring safe, consistent care delivery
  • Scheduling & Coordination
    • Managing rotas and allocating care visits effectively
    • Ensuring continuity of care for clients
    • Managing staff availability and ensuring full coverage of care packages
  • Team Support
    • Supporting and guiding Care Workers in their roles
    • Acting as a point of contact for staff, clients, and families
    • Helping build a positive, supportive working culture
  • Growth & Service Development
    • Supporting the onboarding of new care packages
    • Working across both local authority (council) and private clients
    • Contributing to the growth and development of the service
  • On-Call Support
    • Participating in an on-call rota outside of office hours
    • Providing guidance and support to staff in the field
    • Responding to urgent issues and ensuring continuity of care when it matters most

    • Understanding of service growth and onboarding new care packages
    • Experience working with both council-funded and private clients
