We are expanding our Community Service team at our Worthing Branch, serving customers across West Sussex.

This role is for a Community Care and Support Worker who will support elderly or disabled clients in their own homes.

Responsibilities

  • Complete Client Assessments and develop Client Care Plans.
  • Assist the Registered Manager with hospital discharge coordination and emergency care package requests.
  • Provide medication, personal care, meal preparation, shopping and light domestic duties.
  • Support clients in maintaining independence while living at home.

Qualifications

  • Full driving licence.
  • Valid DBS check and successful referencing.
  • Willingness to work in a caring, warm, friendly and reliable manner – alignment with company values is essential.
