The PCN Pharmacy Technician will improve the quality and safety of care offered to patients of First Coastal PCN based in the east of Lincolnshire. They will support the PCN, Practices and Pharmacy team to ensure safe, effective prescribing and medicines optimisation. We are looking for an ambitious and motivated pharmacy technician with good interpersonal skills to join our PCN team and work alongside our clinical team. As a PCN, we have a great working environment and a great team ethos.
Main duties of the job:
- Support the clinical pharmacists in the delivery of clinical pharmacy services.
- Work as part of the pharmacy and multi-disciplinary team.
- Increase safety and quality of prescribed medicines.
- Assist with medicines reconciliation on transfer between care providers, medication reviews and structured medication reviews.
- Provide support on medicines related issues to the whole PCN.
- Be a point of contact for patients regarding medicines queries.
- Provide remote and/or face-to-face support to patients as needed.
- Be a point of contact for other healthcare providers in the locality e.g. community pharmacies.
- Reduce the medicines related workload on doctors and pharmacists.
- Assist in the management of MHRA/CAS alerts across the PCN.

First Coastal PCN covers a population of approximately 84,000 patients, made up of seven practices. These practices work together across coastal and rural areas to deliver care to the local population. Job responsibilities:
- Ensure safe, accurate and timely supply of prescribed medications for patients.
- Lead on improvements in this area, providing technical and administrative support.
- Support the delivery of good care, helping patients to get the best outcomes from taking their medicines.
- Be the main contact for non-clinical medication related queries from pharmacies, nursing homes and other health care professionals.
- Support implementation of medication changes under guidance of prescribing clinicians.
- Support with audit work in relation to safe prescribing and general quality improvement.
- Work closely with practice administration staff, GPs and pharmacists to answer any queries and take forward any work streams identified.
Responsibilities underpinning the Role:
- Play a role in the delivery of high-quality primary health care services.
- Provide support to individuals, their families and carers to take control of their health and wellbeing.
- Make and maintain contact with local health and wellbeing service providers.
- Refer people to appropriate organisations in Lincolnshire and nationally.
- Present the PCN in cross organisation meetings when agreed.
- Collate feedback/analysis data on behalf of the PCN.
- Maintain a positive, supportive and patient focused culture across the wider team.

Qualifications:
- BTEC Level 3 Diploma in Pharmaceutical Science and NVQ Level 3 Diploma in Pharmacy Service Skills or equivalent.
- Current registration as a Pharmacy Technician with the General Pharmaceutical Council.
- Evidence of commitment to continued professional development.
Experience:
- A minimum of 2 years post qualification experience within Primary or Secondary care.
- Understanding of prescribing and medicines management.
- Up to date clinical pharmacy therapeutic knowledge.
- Knowledge of pharmacy law and ethics.
- Experience of change management and putting ideas into practice.
- Experience dealing with difficult situations.
- Experience of completing clinical audits.
- Experience of working as a pharmacy technician in hospital or primary care.
