- Must be clinically qualified.
- Support a group of CCG’s in the setting up of new Personal Health Budgets (PHBs).
- Implement the next stage of PHBs for the local population.
- Support the Strategic Lead to develop the service moving forward.
- The PHB programme has been expanded beyond Continuing Care in line with the National Strategic direction.
- Use clinical expertise to analyse and challenge health information and assessments to ensure that an accurate assessment of children’s needs are documented.
- Provide high quality, integrated health assessments for children and young people across the area that will determine eligibility for Children’s Continuing Care.
- This is an exciting opportunity to influence the implementation of PHBs to enable children and their carers to have more input to the way their care is delivered.
