Hours: 08:30am - 17:00pm, working 40 hours per week over five days, inclusive of weekend requirements. No on-call responsibilities.

As a Support Assessor, you are responsible for assessing new and existing customers, developing and reviewing person-centred support plans, and ensuring support services are tailored to individual needs, goals, and outcomes. The role involves working closely with customers, families, support staff, and external professionals to ensure high-quality, safe, and effective support that promotes independence, choice, and wellbeing.
Key Responsibilities
- Conduct assessments for prospective and existing customers to identify needs, risks, preferences, strengths, and desired outcomes.
- Develop, implement, and regularly review person-centred support plans that reflect each customer's individual requirements and goals.
- Ensure support plans promote independence, dignity, choice, inclusion, and wellbeing.
- Complete risk assessments and ensure appropriate control measures are in place.
- Review and update support plans promptly following changes in customer needs or circumstances.
- Involve customers, families, advocates, and relevant professionals in the assessment and support planning process.
What We're Looking For
- At least 12 months' experience in care delivery.
- A Level 2 qualification (or equivalent) in Health & Social Care (Desirable).
- Someone who enjoys working in a structured office environment and takes pride in staying organised.
- A strong team player with excellent communication and organisational skills.
- Experience or confidence in writing and reviewing person-centred care plans.
- The ability to build positive relationships and interact professionally with clients, families, and colleagues.
- A flexible, proactive approach and the ability to adapt quickly, as no two days are ever the same.
- Excellent attention to detail and the ability to prioritise effectively.
- A genuine passion for making a meaningful difference in people's lives through thoughtful, high-quality care planning.
