Social Care Assessor required to work in a community social work team based within the Royal Victoria Hospital in Belfast. This role will involve working with the social work team to implement care plans, maintain individual caseloads, and provide advice and support to elderly patients and their families regarding available resources. You will also be responsible for preparing reports and maintaining accurate records, as well as liaising with other healthcare professionals in relation to referrals and ongoing treatment plans for patients.
The hours of work are Monday – Friday, 9am-5pm, with an hourly rate of £14.06. This post will initially last for 3 months, with the possibility of extension.
What We Need From You:- 5 GCSEs grade C or above OR NVQ level 3 in a care related subject
- 1 years’ experience working in a social care OR hospital setting
- To be NISCC registered (or willing to apply)
- Be willing to undergo an Access NI Check
- Full clean driving licence
- Weekly pay
- A minimum of 28 days paid holiday (pro rata)
