Job Description
- Provide therapeutic, recovery-focused care in collaboration with the MDT
- Lead on care planning, keyworker responsibilities, and medication administration
- Support discharge planning and clinical risk assessments
- Maintain accurate patient records and contribute to a positive ward culture
- Participate in handovers, incident reporting, and clinical meetings
