We are seeking an organised, proactive, and experienced Part-Time Care Coordinator to join Feldon Care. The successful candidate will play a key role in coordinating care services, supporting care staff, and ensuring the delivery of high-quality, person-centred care.
Key Responsibilities:- Coordinating and scheduling care visits efficiently
- Managing staff rotas and ensuring adequate care coverage
- Liaising with clients, families, healthcare professionals, and care staff
- Conducting care reviews, assessments, and care plan updates
- Monitoring service quality and supporting compliance with CQC standards
- Assisting with recruitment, onboarding, and staff support
- Maintaining accurate records and care documentation
- Responding to day-to-day operational and service delivery needs
- Previous experience as a Care Coordinator within domiciliary care or a similar health and social care setting
- Strong organisational and time-management skills
- Excellent communication and interpersonal skills
- Good understanding of care planning, safeguarding, and compliance requirements
- Ability to work independently and manage multiple priorities
- Competent in using Microsoft Office and care management systems
- Up-to-date Mandatory Training certificates
- Right to work in the UK
- Enhanced DBS (or willingness to obtain one)
- Full UK driving licence and access to a vehicle desirable but not essential
