The post holder will work across a network of primary care practices in Buckinghamshire, supporting medicines optimisation and delivering high‑quality pharmaceutical care. This role plays a central part in ensuring safe, effective, and evidence‑based prescribing for patients with long‑term conditions.

Responsibilities

  • Undertake structured medication reviews for patients with complex polypharmacy.
  • Manage medicines for patients with chronic conditions and support care homes and house‑bound patients where appropriate.
  • Improve prescribing safety and maximise cost‑effective prescribing to enhance patient care.
  • Provide specialist advice on medicines, addressing public health and social care needs.
  • Work as part of a multidisciplinary team in a patient‑facing role to assess and treat patients using expert knowledge of medicines.
  • Develop and implement medicines management plans in line with local and national guidance.
  • Build strong relationships with pharmacy professionals across PCNs and the wider healthcare system.
  • Contribute to shared care protocols, clinical research, and liaison with specialist pharmacists, community pharmacies, and anticoagulation services.
  • Receive support, mentorship, and training from a Senior Clinical Pharmacist, and participate in dedicated GP supervision time.

Qualifications

Essential

  • Registered with the GPhC.
  • MPharm degree (or equivalent).
  • Successful completion of the CPPE Primary Care Pharmacy Education Pathway.
  • Experience in primary care or within a Primary Care Network, including medication or medicine use reviews and patient counselling.
  • Experience of working with clinicians and multidisciplinary/multiagency teams.
  • Experience of facilitating change to improve clinical practice.
  • Experience of delivering training sessions to varied groups.
  • Experience of undertaking clinical audit.

  • Has previously worked in a primary care setting or a Primary Care Network.
  • Ability to listen, empathise, and provide person‑centred support in a non‑judgemental way.
  • Courteous, respectful, and helpful at all times.
  • Able to engage with people from all backgrounds and communities respecting lifestyles and diversity.
  • Commitment to reducing health inequalities and proactively reaching people from all communities.
  • Ability to use discretion and sensitivity.
  • Ability to work on one’s own initiative.

  • Ability to travel between sites in a timely manner if required.

Optional: Independent Prescriber or qualification from CPPE Clinical Pharmacist training pathway.
