The post holder will work across our network, which includes three practices: Haddenham Medical Centre, Cross Keys Surgery, and Unity Health. Our surgeries are based in Princes Risborough, Chinnor, Haddenham, Thame, Brill, and Long Crendon. The role will play a central part in supporting medicines optimisation and delivering high-quality pharmaceutical care across the network.
Responsibilities
- Supporting patients with long‑term conditions
- Undertaking structured medication reviews
- Managing medicines for patients with chronic conditions
- Supporting care homes and housebound patients, where appropriate
- Improving prescribing safety
- Helping patients to achieve the best outcomes from their medicines
- Responding to prescribing queries
Qualifications and Experience
- GPhC registration
- MPharm degree (or equivalent)
- Successful completion of the CPPE Primary Care Pharmacy Education Pathway
- Experience in Primary Care or within a Primary Care Network
Support and Development
You will receive support, mentorship, and training from a Senior Clinical Pharmacist, along with dedicated GP supervision time.
