At FCMS we provide several services across the Fylde Coast, MorecambeBay, Rossendale, West Lancashire and Doncaster as well as diagnostic servicesacross the country. These include GP Out of Hours services, Urgent TreatmentCentres, and range of other Primary Care Services including Enhanced Access(Fylde Coast) and Special Allocation Services (Lancashire / Cumbria/Parts ofCheshire and Merseyside).
TDM supports people across Blackpool to address clinical and non-clinicalfactors increasing their risk of an unplanned hospitalisation. Based on theevidence-base there is an underlying risk factor of socio-economic disadvantagewhich means that more support delivered through non-traditional mechanisms isrequired to improve health outcomes.
Key aims of TDM are to provide clinical and non-clinical interventionsthat meet the needs of individuals identified in Blackpool through a mixedcase-finding approach including the Core20Plus5 cohorts and with the followingLong-Term Condition β cardiovascular disease (CVD) and/or COPD. The aim is tosupport them to achieve their maximum potential for physical, cognitive, socialand psychological function, participation and quality of living. To achieve allthese goals, we will be working collaboratively withother Blackpool Based organisations such as Healthwatch, PCN SocialPrescribers, Community Nursing and Housing Intervention teams.
Main duties of the job
We are looking for motivated clinicians to deliver high-quality,patient-centred nursing care within the heart of the community on our mobileunit. The role focuses on patients withlong term conditions, particularly cardiovascular disease (CVD) and COPD withinthe core20plus5 cohorts. The postholderwill provide holistic clinical and non-clinical interventions aimed atimproving physical, cognitive, reducing health inequalities and preventinghealth crisis.

Base: Work will be undertaken on our MobileUnit with a base of Newfield House, Vicarage Lane, Blackpool, FY4 4EW
Day to Day Duties to include, but notexhausted:
To provide proactive reviews and support for patients with CVD and / orCOPD in line with clinical competence and local pathways
Undertake holistic health assessments, identifying physical, mental andsocial needs
Deliver health prevention and health promotion interventions includinglifestyle advice (alcohol, smoking, diet and physical activity), weightmanagement and wellbeing support, Opportunistic screening and vaccinationadvice
Identify unmet needs and escalating health concerns, taking appropriateactions
Deliver care within the mobile clinical unit adapting consultations to acommunity-based environment
Support people who present to the mobile unit by providing assessment andadvise where appropriate, signposting and referring to the most suitableservice when out the scope of practice and offering reassurance, healthinformation and ongoing support
Promote self-management and empower patients to engage with healthservices
Accurately record all activity, assessments and outcomes on EMIS usingappropriate clinical coding and templates
Maintain confidentiality and data protection standards
Maintain Current NMC registration adhering to the NMC code of conduct
Work autonomously withing scope of practice, escalating concernsappropriately
Participate in supervision, appraisal and continuing professionaldevelopment
Contribute to service development, evaluation and quality improvementinitiatives
