We are looking for a Registered Nurse (Wound Care) to join our Wound Care Service. The service is spread across clinics in Fleetwood, Freckleton and St Annes.

Pay: Β£15.88 - Β£19.33 per hour depending onexperience

Accountable to: Lead Wound Care Nurse

Reports to: Clinical Manager forLancashire Place Based Care

Main duties of the job

The post holder will undertake wound assessments and treatment for patients with simple wounds. Patients who will be referred to this service will be the following types:

Post operative including suture and staple removal

Post trauma skin tears, self-harm

Acute wounds that require follow up

Cavity Wounds

Burns/scalds that require follow up

Patient education including nutrition in relation to wound healing and scar management

There is an expectation to refer to the wider community team if required, support Senior clinicians and HCAs and attend meetings when required. The post holder will help promote the service and implement change as requested from the ICB.

Exclusion Criteria: Those patients who are: Not registered with a GP in Fylde and Wyre CCG (i.e., reside outside area) House bound Undergoing or requiring Vac Therapy patients Undergoing or requiring Larvae Therapy Doppler patients

Job responsibilities

DUTIESAND RESPONSIBILITIES

Endeavour to maintaingood standards of communication both in written and oral form.

Assess a wound andmake appropriate dressing selection for the desired plan of care within localformulary and national guidelines.

Prescribe safely within the prescribingcompetency framework and undertake prescribing audits relevant to service(relevant to Non-Medical Prescribers)

To assess patientsautonomously whilst remaining within scope of practice

Work within formally agreed arrangementswhere general practices provide specialised or extended clinical services (LES:Local Enhanced Services)

Show awareness ofevidence-based practice in wound care

Improveaccess to wound care services

Provideshort-term care of acute wounds Provide continuity of care and a consistentapproach

Providea comprehensive wound care management service Standardise adherence to the ICBs/ Lancashire and South Cumbria wound care formulary working towards achievingan 80% compliance rate

Improveskills and knowledge of the workforce leading to a higher standard of care

Enhancepatient outcomes and reduce secondary care activity

Participate inpromoting and implementing services to new sites when required by the ICB forthe expansion of the service

Make entries onpatient records to record clinical interventions

Make entries in theclinical notes using the service areas computer programme ADASTRA

Complete associateddocuments to accompany specimens using Cyberlab where available and labelspecimens

Work well within asmall team

Be flexible withworking pattern across the current locations to assist with teams Annual leave/ sickness to ensure the service is covered at all times

Communicate withpatients and carers in a polite and pleasant manner

Receive and deliverpatient related information to and from other departments via the telephone/emails

Educationand In Service Training

Be fully conversantin all the clinical computer systems used within the Wound Care Service

Attend relevantstudy/induction days, seminars, courses, self-development etc. for individualdevelopment and for the benefit of the service. Keepup to date with mandatory training.

Where necessary relevant training in the operation of new orunfamiliar equipment, software or procedures will be provided or arranged.
