Main duties of the role:

TDM supports people across Blackpool to address clinical and non-clinical factors increasing their risk of an unplanned hospitalisation. Key aims of TDM are to provide clinical and non‑clinical interventions that meet the needs of individuals identified in Blackpool through a mixed case‑finding approach including the Core20Plus5 cohorts and with the following Long‑Term Condition – cardiovascular disease (CVD) and/or COPD. The aim is to support them to achieve their maximum potential for physical, cognitive, social and psychological function, participation and quality of living.

To achieve all these goals, we will be working collaboratively with other Blackpool Based organisations such as Healthwatch, PCN Social Prescribers, Community Nursing and Housing Intervention teams.

We are looking for motivated clinicians to deliver high‑quality, patient‑centred nursing care within the heart of the community on our mobile unit. The role focuses on patients with long term conditions, particularly cardiovascular disease (CVD) and COPD within the core20plus5 cohorts.
