We are looking for a Care Navigator to join our Care Coordination Team!
Contract: Permanent
Hours: Part-time
Location: Newfield House, Vicarage Lane, Blackpool, Lancashire, FY4 4EW
Salary: £13 an hour Weekend and Overnight Enhancements Apply
Main duties of the role:
- The post holder will provide comprehensive administrative support to the Care Co-ordination Service, ensuring a fast, accurate and effective telephony service for patients and healthcare professionals.
- They will support clinical staff with a range of administrative tasks, including the accurate recording and maintenance of patient information, such as end-of-life care records and long-term health conditions.
- The role requires the ability to work calmly and professionally under pressure whilst remaining sensitive to the needs of patients and callers.
- The post holder will be responsible for handling confidential and sensitive information in line with Information Governance policies, maintaining patient confidentiality at all times.
- They will undertake a variety of clerical, administrative and data entry duties, following established protocols and procedures, and will be expected to complete all relevant training associated with the role.
- The role also involves working closely with partner organisations, including Trinity Hospice, North West Ambulance Service, Blackpool Teaching Hospitals, GP Practices, and neighbourhood teams across Blackpool, Fylde and Wyre, to support the delivery of high-quality patient care.
