Post: Advanced Clinical Practitioner Inclusion Health Pay: £28.48 - £31.73 depending on qualifications and experience Hours: 21.5 hours per week CLOSING DATE: 14 JUNE 2026 * Please note that our roles are not being considered for sponsorship at this time* We are looking for individuals with a passion and enthusiasm to work with some of the most vulnerable and complex people within our community. The right candidate should be approachable and display excellent communication skills, compassion and a non-judgemental attitude. We are looking for an advanced clinical practitioner to join the team who can work autonomously within the special allocation scheme, as well as the wider inclusion health services. Main duties of the job Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue. Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support through out the week, outside of the ward round, during opening hours.
