Would you like to join our friendly and resolute team of clinicians and non-clinicians at a large NHS general practice based in Slough, Berkshire? We are looking for a motivated and experienced Clinical Pharmacist to join our team. You will be part of a team of five Clinical Pharmacists (FRMG & PCN SPINE) providing multi-disciplinary support to the practice and its patients through primary care. You would be based at Farnham Practice but would be expected to travel to our other locations and our PCN SPINE partner, Kumar Medical Centre, within Slough, to be involved in the wider running of the practice, such as participating in clinical meetings and contributing to the development of new services. You will be part of the Clinical Pharmacist team ensuring prescribing medication is safe, effective, and is optimised for and with our patients.
Main duties of the job:
- To work closely with SPINE PCN GPs and act as point of contact in the practice for all medicines related queries for health care professionals and patients.
- To manage the repeat prescribing reauthorisation process and undertake medication reviews of patients, some of which may need to be undertaken in the patients home.
- To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
- To support the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF and Locally Commissioned Services.
- Manage practice formularies to improve the quality, safety, and cost-effectiveness of prescribing.
- Where appropriate, review pathology results for patients on known medicines.
- To help plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing and undertake medication audits.

Farnham Road Medical Group (FRMG) started as a small surgery and have grown over the years into an outstanding large training GP practice that is approved by Health Education England. With four clinical sites based in Slough, we have a large Multi-Disciplinary Team (MDT) looking after approximately 38,730 patients. The practice is part of SPINE Primary Care Network (PCN) and are committed to innovation in providing high quality health care for our patients and a protected working and learning environment for our staff.
Our MDT team includes GPs, pharmacists, nurses, physician associates, paramedics, MSK practitioners, Mental Health Practitioners, managers, social prescribers, and care coordinators supported by a large Patient Services team. This resolute team provides an evolving healthcare and medical service to meet the varying needs of our patients and, in turn, provide what we believe is a sustainable model of modern General Practice.

Job responsibilities:
- Working closely with SPINE PCN GPs to resolve day-to-day issues related to medication.
- Acting as point of contact in the practice for all medicines related queries for health care professionals and patients.
- Managing the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
- Undertaking medication reviews of patients including those with multiple long term conditions and polypharmacy to improve patient outcomes and maximise cost-effective prescribing.
- Ensuring that the practice repeat prescribing policy is regularly reviewed and that it is being adhered to with the aim of reducing waste and improving safety.
- Contributing to reductions in medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues.
- Supporting patients recently discharged from the hospital by ensuring they understand their medication, any changes that have happened recently and have a timely supply.
- Facilitating good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
- Following up issues highlighted by Medicines Use Reviews carried out in local Community Pharmacies.
- Supporting the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF, Locally Commissioned Services and the QIPP agenda.
- Undertaking minor ailment clinics or triage.
- Managing practice formularies to improve the quality, safety and cost effectiveness of prescribing.
- Logging and actioning drug withdrawals and alerts e.g. monitoring CAS and MHRA alerts aimed at improving medicines safety.
- Where appropriate, reviewing pathology results for patients on known medicines.
- Qualifying as a prescriber if this has not already been done.
- Helping plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing.
- Producing patient information leaflets and posters and running medicine awareness projects throughout the year.
- Attending the CPPE training and development program.
- Awareness of and compliance with all relevant practice policies and guidelines.
- Reporting on Significant Events.
- Contributing to the development of computer-based patient records on EMIS Web.
- Contributing to the summarising of patient records and SNOMED-coding patient data.
- Participating in regular staff training sessions to ensure development of skills and knowledge; to assess own learning needs and undertake learning as appropriate.
- Participating in clinical, multi-disciplinary and whole team meetings.
- Observing and complying with all appropriate statutory requirements in relation to Child and Adult Safeguarding and to be familiar with the reporting process should this be required.
- Contributing to policy development and implementation of procedures and guidelines in relation to their own area of work.
- Attending any Protected Learning Time sessions provided by either the CCG for the Practice or by the Practice itself.
Person Specification:
Experience:
- To be a qualified Clinical Pharmacist with a minimum of two years experience in the Pharmaceutical Industry.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a team and perform effectively under pressure.
- Be highly motivated with excellent clinical timekeeping.
- Ability to consistently observe and comply with all appropriate statutory requirements in relation to Child and Adult Safeguarding and to be familiar with the reporting process should this be required.
Qualifications:
- Qualified Pharmacist and registered with GPhC.
- Enhanced DBS check is required.
