The ideal GP candidate will work autonomously and as part of a team within the Same Day Service at Farnham Integrated Care Services. The service provides high-quality medical care to the Farnham PCN population.
Main duties of the job
- The role requires delivering excellent acute clinical care whilst adhering to General Medical Council (GMC) standards and contractual obligations, fostering positive collaboration within the multidisciplinary team at FICS and the Farnham PCN Practices. This role will not case manage ongoing patient care, review test results or provide follow ups.

Job responsibilities
- Clinical Care
- Deliver high-quality medical care to the PCN entitled population, with a focus on acute same-day care and integrated services.
- Prescribing in line with local and national guidelines.
- Conduct telephone triage for patient assessments and care recommendations.
- Maintain accurate and compliant clinical records, following best practice and guidance.
- Administrative Responsibilities
- Process referrals and other related administrative tasks promptly.
- Perform regular quality checks of referrals and feedback.
- Team Collaboration and Development
- Work collaboratively within the MDT, sharing clinic workload equally and promoting a positive, cooperative work environment.
- Support the training and mentoring of various clinical roles.
- Attend and contribute to team meetings, actively supporting the development and continuous improvement of clinical governance and service performance.
- Quality Improvement and Service Development
- Lead and participate in quality improvement initiatives, ensuring the service adheres to clinical guidelines and follows best practices through audits and feedback.
- Encourage a culture of continuous improvement, collaborating with all staff and liaising regularly to enhance service efficiency.
- Engage in strategic planning with directors, aiming to improve clinical performance and resource utilisation.
- Resource Coordination
- Support clinic coordination to ensure effective resource use, delivering high-quality service.
