Role Overview

Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region.

Our team has grown and evolved to meet the changing needs of the community and we are looking for a highly motivated Registered Nurse Plus/HCP Plus to join our Community Teams.

Responsibilities

  • Deliver excellent specialist palliative care within a multidisciplinary team to patients and their families in mid Essex.
  • Act as team role models and be part of developing new ways of working, alongside quality improvement.
  • Manage caseloads and undertake assessments including MCA.
  • Provide excellent symptom control, psycho‑social and spiritual support for patients under the care of the community teams, supporting patients and significant others through to end‑of‑life care.
