Farleigh Hospice is seeking a senior leader to head the Hospice Rapid Access Service (HRAs) in Chelmsford. You will oversee a team of clinicians and care coordinators, coordinate rapid discharge or community care, and ensure compassionate, coordinated end‑of‑life support.
Reporting to the Director of Care, you will drive performance, manage complex cases, and collaborate with community partners, the acute trust, the ICB and Adult Social Care.
