Farleigh Hospice is seeking a Care Coordinator to lead the procurement of nursing home placements and/or domiciliary care for patients assessed for Hospice Rapid Access Service.
You will develop relationships in-house and with external organisations to ensure patients have access to the most appropriate care environment, manage timescales sensitively and uphold GDPR and confidentiality.
