Job Advert

The Leaving Care Team provide advice and guidance to young people who are, or have been, looked after away from home by Falkirk Council aged 16 to 26. This is a permanent post

Work as part of a team to provide support and guidance to young people in the transition from care to more independent living.

Develop and maintain key worker relationships with individual young people.

Work in partnership with Fieldwork staff and colleagues from other agencies to contribute to and assist in the implementation of agreed Pathways Plans.

Hold case responsibility for young people in continuing care placements and older young people in receipt of an aftercare service.

Co-ordinate, prepare and present reports for all relevant meetings and reviews in respect of allocated cases and maintain recordings of all client related contacts.

Engage in inter-disciplinary team work with care, education, housing, criminal and youth justice, health providers and any other agency involved in the young people’s life, to best meet their needs.

Work as part of a team to support young people to access and sustain accommodation appropriate to their needs.

Encourage and support young people to realise their potential by participating in appropriate opportunities including volunteering, education, training and employment.

Support young people to maximise their income through activation and maintenance of benefits and to manage payment of rent, council-tax and other bills.

Support carers to implement planned programmes relevant to the needs of young people.

Contribute to the ongoing development of the Council’s Leaving Care Service.

To participate in Continuous Professional Development.

You must have the ability to be mobile across the Council area.

ESSENTIAL:

  • HNC in Social Care and SVQ3.
  • Demonstrable evidence of working with young people who have experienced trauma and/or who have been looked after away from home

If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
