Job Description\ N Job Advert\n Grahamston House is resource centre/care home for older individuals living with dementia and associated physical health conditions.\ N You will be responsible,as part of a team, for the provision of high-quality person-centred care for each individual. Have knowledge and experience of good care practice in meeting the needs of older people with dementia and those who are clinically frail. \ N The successful candidatewill demonstrate skills in assessment, support planning and recording. You will hold excellent communication skills, be experienced in multidisciplinary working, demonstrate a flexible and positive approach to ensure the best outcomes for the individuals we support.\ N An SVQ 3 in Social Care is desirable and you must be able to register with the SSSC and qualify within the regulatory timescale of three years.\ N This post is temporary until March 2027 due to service requirement. \ N You will work 33.25 hours per week, on a nightshift rota basis including working weekends.\ N If you are the successful candidate, you will be required to gain and maintain PVG scheme membership, the cost of which will be met by the preferred candidate.\ N \n
