Due to expansion of the business, a fantastic opportunity has arisen for an organised and experienced Care Co-Ordinator to join our team. Falcon-Care is a leading provider of specialist care in the North, currently recruiting for a Care Coordinator to join our team in Blackburn.
The Care Coordinator is responsible for the effective management and supervision of care workers and supporting the Branch Manager to ensure the branch is providing an excellent provision of service to our customers, clients, and colleagues on a daily basis. This includes ensuring compliance with national minimum standards and using effective rostering to fully utilise staff hours and drive employee engagement.
What the role entails:
- Allocation of care workers to customer visits by using effective rostering, ensuring continuity of care and taking into account locations, skills required, and customer preferences.
- Organise care worker rotas to minimise changes to the service and travel between customers' homes.
- Work with the management team in recruiting suitable applicants according to business needs.
- Take responsibility for new referrals from private customers and social services teams, ensuring full information is received and appropriate care planning is undertaken.
- Assist Branch Manager with identification of further business opportunities outside of current client base.
- Match care workers to customers, ensuring care workers are notified of all conditions relating to the assignment.
- Support with the on-call out of hours service on a rotational basis as agreed with your manager.
- Be able to deliver care as part of the emergency on-call team, should the situation occur.
Essentials for the role:
- You must possess or be willing to work towards a minimum of Level 3 in Health and Social Care or equivalent.
- You must hold a full UK driver's license and have access to your own vehicle.
- You must be able to work in a fast-paced demanding job.
- You must be able to meet critical deadlines and work towards targets.
- You must have strong ICT skills.
- Previous experience in a home care coordinating role.
Preferred but not essential:
- Knowledge of Care Standards.
- Knowledge of CQC legislations and Inspection process.
