About the service:
Extern’s Multidisciplinary Homeless Support team (MDHST) has operated within the Belfast area since 2006; since then, it has provided a specialist service for individuals and families who are homeless/at risk of homelessness and who also experience additional complex needs including addictions, mental health difficulties, learning disability, and family breakdown. The team offers a range of professional interventions and has developed successful partnerships with core agencies across the statutory, voluntary and community sectors to advocate for appropriate service provision and support planning with the aim of realising the potential of each service user. The approach utilised to promote change encompasses a high tolerance, low threshold methodology including the principles of harm reduction.
This post will be based within our Floating Support service which involves working with people with complex needs and those who have recently been granted Refugee status in Northern Ireland. As a key worker you will offer a pragmatic and holistic assessment, care planning and structured ongoing support to people who are in their own accommodation and integrating into the community. The post involves working in partnership with service users in their home environment and includes a wide range of practical assistance, evidence-based interventions, advocacy and liaison with both statutory and voluntary agencies and the ability to assess, manage and work effectively with risk.
