The Role
As a Support Worker, you will
- Provide person-centred care and support in line with individual care plans.
- Support individuals with personal care, medication and daily living.
- Encourage independence, choice and decision-making.
- Support individuals to access the community and develop social relationships.
- Promote healthy lifestyles and wellbeing.
- Work closely with Team Leaders, Care Managers and other professionals.
- Contribute to person-centred plans, reviews and support goals.
- Act as a Key Worker where required.

Previous Support Worker experience is preferred but not essential.
